After printing and filling out the form, please sign on the last page and fax all pages to the toll free number at the top of the form.

If you do not have a fax, Walmart or an office supply store can assist with a fax or you can scan and email the completed form with your actual signature on it.

A signed inspection agreement is required by my insurance company and the state.
